What to Bring to Your Appointment

At the MUSC Health Advanced Migraine Surgery Program, we understand that your time is valuable. We have therefore provided several forms below that you can complete prior to your visit in order to make your initial meeting with us as productive and efficient as possible.

1. Open, print and complete each form below.
2. Return a copy of each of the forms to us prior to your appointment (you may send via fax at 843-792-3080).
3. Please make every effort to also bring the original copy of the completed forms with you to your first appointment.

• Prior Medical Treatment Team (PDF) (list of physicians and medical providers who have previously treated you for migraine headaches and difficulty breathing through the nose)

• Functional Nose Information Sheet (PDF)
• Pre-treatment Questionnaire (PDF)
• Post-injection Questionnaire (PDF) (for those receiving Botox injections)
