Post Offer Health/Drug Screening

Your Human Resources recruiter will email you links to important health screening forms. Prior to your appointment, the forms must be completed and submitted to Employee Health Services. Failure to do so may delay your employment date. Note: Your job offer will be rescinded if you fail to complete the drug screening during the Employee Health Services appointment.

What do I bring to my appointment?

  1. Drivers license or state issued ID
  2. List of prescription medications you are taking
  3. Current immunization records
  4. Official documentation proving/showing receipt of flu vaccine IF you already received your flu vaccine/flu mist for the current flu season. This will ensure compliance with our compulsory flu policy and not delay your start to work date.

What to Bring on Your First Day

  2. Social Security Card. This is required for payroll processing. If you are using the social security card as one of the required documents for the I-9, an original is required. If you are providing the social security card for Payroll purposes only, a legible photocopy is acceptable - Additional information on social security cards (PDF)
