Who We Serve

Coastal Connections will consult with MUSC patients served in value-based programs such as MUSC's Medicare Accountable Care Organization. Our social worker assists with many different types of needs resulting from various circumstances.

Areas of Assistance 

Food Assistance

• Referrals to those who can help with SNAP enrollment
• Referrals to local soup kitchens and food pantries

Utility Assistance & Income Support

• Referrals to those who can help with financial counseling
• Referrals to organizations that can assess financial situations and help pay bills (if qualified)

Housing Search

• Referrals to housing shelters and low-income apartments
• Referrals to organizations that can assess individual needs and help you find the best housing

Employment

• Referrals to different agencies for job and life skills training
• Referrals to GED courses
• Referrals to agencies to do job matching

Transportation

• Referrals for reduced cost public transportation (if qualified)
• Referrals to transportation services for medical appointments

Clothing/Home Goods

• Referrals to clothing pantries and discounted clothing stores
• Goodwill vouchers
• Referrals to furniture banks, if qualified

Child Care Resources

• Referrals to affordable child care centers

Other Assistance

• The Community Navigation Social Worker will do any research in order to fulfill a client's needs that can be solved through community resources
